How to Get Started with Foundations OT

Step 1: Call your physician
Contact your primary care doctor or nurse practitioner to ask for an "outpatient OT " order. Ask them to fax the order and pertinent diagnosis information to our office at 605-305-3643.
*If your physician hasn't heard of Foundations, let them know we are "mobile outpatient OT company specializing in working with older adults in their homes".
Step 2: Call our office
After you have completed Step 1, call our office 605-705-0254 to provide us with your information so we can verify insurance coverage and discuss your request for OT services.
Step 3: Insurance coverage
While it is ultimately your responsibility to understand your insurance coverage and financial responsibility for outpatient therapy, we will inform you if we are out of network with your plan and how that may impact your financial responsibility.
Step 4: Schedule initial evaluation
Once your insurance has been verified, the office will work with you to schedule your OT evaluation. After the first visit, you will schedule follow-up visits directly with your OT.
Step 5: Collecting payment for patient responsibility
Accepted insurance plans will be billed at the end of each session. If we are aware of a co-pay for each visit, we will collect payment at each session with a debit or credit card. If we aren't sure of the patient's responsibility for each session, we'll wait to collect payment when all insurances have paid their portion and determined an amount due by patient. A statement with charges and payment received will be provided upon request.
